The difference between form I693a and form I693

While processing my (AOS), I had a lot of trouble getting the required documents because I wasn’t able to differentiate the form I693a from form I693. Instead of downloading the form I693a, I downloaded the I693 and that was a huge mistake.

Though both of this forms are used for medical purposes usage, there is a big difference between the two. Form I693 is the form used to get the “whole medical exam” while the form I693a  (or known as the Vaccination Supplement form) is for medical result transfer (a certain AOS applicant took from his/her country) purposes only.

There was only one medical process that a fiancee visa holder is lacking, that’s the TD-shot. I took my TD-shot in the base (my husband works in USAF) mainly because it’s accessible plus the fact that it’s free. I heard that TD-shot cost around $25 and up depending on where you would get it.

Anyway, after taking my TD-shot, they then furnished me a copy of my TD-shot record. Along with this record and my medical records from Saint Lukes, I submitted it to the civil surgeon for tranferring of my medical records to the form I693a. Now, AOS only requires the applicant to submit the I693a form (along with other requirements) in a sealed envelop from civil surgeon.

So it is very important for you to know ahead of time the difference between form I693a from form I693. I run a search today and here are the links on where to download both these forms:

i am about to submit my aos package. are you sure that i only need to submit the I-693a? i do agree with you and as far as i know too, that it’s all i need together with the rest of my aos package.

however, when you look at I693 instructions- it says something like

you don’t need a medical anymore but you are required to fill out PART 1 and the civil surgeon must complete part 5. am confused. pls help.

Hi MW and acg,

The answer to your question is YES. As far as I know and my experience is concern, I only submitted the form I693a or the vaccination supplementary form. You DO NOT need the I693 form because you’re already done with the medical exam.

If you are still confused, read the guideline from the form I-485. It’s all written in there.

hi…i just went to a registered civil surgeon but he asked me to have TD, Varicella and Human Papillomavirus to get done…he recomended a clinic where i can have it all done then he told me to just come back if i am already done with all the vaccination he required…i thought we only need the TD shot? ‘coz it’s in my DS-3025 that i am already done with the Varicella and Human Papillomavirus..’coz St. Lukes gave me 3 shots: MMR, and the other two were the Varicella and Human Papillomavirus so how come that i have to take the other two still? waaaaaahhhh…Varicella and Human Papillomavirus are so expensive….the two would cost me like around $225…huhuhuh! why?

Did you took it? I hope you did not because you only needed the TD shot on which you are right. That civil surgeon do not know the ‘real’ process for k1 visa.
I’ve also encountered that type of civil surgeon before BUT I did not took the tests that he asked me, instead my hubby and I went back home and I started calling the civil surgeons assigned in my area.

It took me 15 calls before I was able to find a civil surgeon who knows the process. That is one technique that I always encourage my friends to do. It will save you time, money and confusion. You can do that too. The only thing that the civil surgeon needs to do with our medical record is to TRANSFER OUR MEDICAL RECORD TO THEIR OWN RECORD and SEAL it. This sealed envelope (along with your I-485 papers) will be sent to USCIS.

Hi, im preparing my AOS now and my only problem is my medical record. I did went through that confusion…and I ended up having all the shots all over again. Now my problem is, they gave me the signed I-693a without putting it in a sealed envelope. Does it have to be really sealed or is it ok to submit the signed I-693a form unsealed anyway? Thanks so much…

Vaccination do not need to be in a sealed envelope like x ray image is not sealed.Only medical tests needs to be confidential.

Im in the same situation right now. I just want to know where did you get the link to the form I-693a?I searched for it at the USCIS website and its not in there…anyway,I have a friend in here who is processing her AOS too,and just like most of you guys,she had another three shots. But the only difference is,she didnt submit an I-693 or any medical form together with the I-485 packet. The USCIS just asked for her to bring the medical forms on her interview next month…and I did the same,I submitted the packet without any medical forms…What bothers me now is that if I will download and fill out the I-693a form from this site and will not have more shots,is it guanteed to be of no trouble during the interview?Thanks…
